what you need to know

  • The Share the Care program is our way of saying thank you for sharing the positive experience you’ve had at Concierge Medicine of Westlake. When you refer a friend or family member to our practice, both you and the new patient will receive a $100 credit toward your next membership renewal.

  • The referral program is open to all current patients of Concierge Medicine of Westlake.

  • Your referral must be someone who is not currently a patient at our practice and who joins as a full-paying annual member.

  • Patients can refer friends or family by:

    • Picking up a referral card at our office and sharing it with their referral.

    • Submitting a referral form on our website.

  • Once your referral has completed their first appointment and becomes an active member, you’ll receive a $100 credit applied to your next membership renewal. The new patient will also receive a $100 credit.

  • Absolutely! There is no limit to how many people you can refer. Credits will stack, allowing you to save even more on future renewals.

    • Referral credits cannot be applied retroactively.

    • Credits are non-transferable and not redeemable for cash.

  • This program is available for a limited time and may be modified or discontinued at any time at the discretion of Concierge Medicine of Westlake.
